Point Value as assigned: Scale 1 to 4 points



1= Strongly Disagree 2= Disagree 3= Agree 4= Strongly Agree (Except questions 3,5, 7,9 & 12 where value is reverse)



Ex. Strongly Agree would now equal 1pt. instead of 4pts.
